Hospice care is about more than end-of-life care. When you or a loved one are facing a life-limiting illness, our place is by your side with a unique and personal plan of care to address the emotional, spiritual and physical needs of both patient and family during this tender time in life.
Colquitt Regional Hospice enables your loved one to spend his or her last days with dignity in the comfort of home, surrounded by beloved family or friends. Our services are available 24 hours a day, 365 days a year, across Colquitt County and many surrounding communities and counties.
Services
Colquitt Regional Hospice care, provided by an interdisciplinary team of clinicians and professionals, is available to patients seeking palliative treatment with a life expectancy of six months or less.
Our registered nurses work closely with your interdisciplinary care team to render and oversee your care at home, at your assisted living home or at nursing home facilities. For emergencies, a nurse is on call 24/7.
With a personalized approach, we coordinate selecting, dosing and monitoring medications to help patients feel comfortable and manage pain and other symptoms.
Our home health aides can assist with activities of daily living, personal care (such as bathing, dressing and grooming) and other comfort measures. We can also perform light household tasks or errands outside the home.
When needed, we will facilitate your transition to short-term inpatient care, including respite care that provides relief for your family caregivers.
Spiritual care is essential to our holistic approach to care. We listen to your spiritual and religious concerns and help to explore fears and find peace and meaning during this tender time.
Your social worker will help with the social, emotional and practical challenges related to your illness. Services may include counseling, connecting you to community resources, assistance with advanced care planning or providing bereavement support.
Our volunteers can provide companionship and support for you and your family. They can help with errands or light household chores, or provide a listening ear, read books and play games, while also assisting our professional staff.
Emotional and psychosocial support for loved ones of a patient nearing the end of life, as well as after death, is an integral part of our services to help families cope with grief and loss.
