Email a Patient

Have a friend or loved one in the hospital at Colquitt Regional Medical Center? Use this form to send them a message of support! All messages are printed in one of three design motifs, based upon your selection: Thinking of You, Get Well Soon or Congratulations. They are then hand-delivered to patients by our staff of volunteers.

PLEASE READ THE FOLLOWING ABOUT THE “E-MAIL A PATIENT” SERVICE:

Please complete all the required fields so that we can ensure that your message is delivered promptly. 

Your message will be delivered by our volunteers during normal business hours, Monday – Friday, from 8:00 a.m. – 5:00 p.m.

Please note that delays in delivery may occur, especially during weekends and holidays. We will make every effort to deliver your message promptly – but we cannot guarantee delivery. If a patient cannot be located, we will attempt to contact the sender; however, this may delay delivery. If a patient has already been discharged, we will attempt to contact the sender and the message will be discarded. Undeliverable messages will be discarded.

The E-mail a Patient service should not be used for confidential communications – it is meant solely as a method of sending encouragement to patients in the hospital. Please do not include information of a personal nature or include any medical or diagnostic information. All messages are read and printed by our volunteers.

Inappropriate messages, including business solicitations or messages containing offensive or obscene language, etc. will be discarded at the discretion of the volunteers.

Unfortunately, we can only receive and deliver messages at this time – we are not able to provide a response back from the patient or provide outgoing E-mail services for patients. Patients may be contacted directly by telephone by calling the main switchboard at (229) 985-3420.
