Community Health Needs
Community Health Needs Assessments (CHNAs) help us better understand the unique health needs of our communities and identify gaps, assets and resources. Working with a diverse group of community leaders and the Georgia Health Policy Center, we can collect comprehensive data and use our learnings to plan, manage and evaluate community health initiatives.
CHNAs pinpoint the most vital health needs of the community, informing our next steps and helping us create a blueprint for improving care and promoting healthy lifestyles. An implementation plan outlines how to address unmet needs in the coming years.
Assessment documents are available below in compliance with the Internal Revenue Service and the Affordable Care Act (ACA) tax law requisites section 501(r), requiring not-for-profit hospital facilities as described in Code section 501(c)(3) to conduct and share a CHNA once every three years.
