We coordinate care for your inpatient stay.
Hospital Medicine is a medical specialty dedicated to comprehensive care for hospitalized patients. Hospitalists are available 24 hours a day, 7 days a week. With a deep understanding of the unique needs and challenges of inpatients, hospitalists will plan and oversee treatment throughout your inpatient stay to help you achieve the best outcome possible. They will facilitate collaboration and communication with your primary care physician and across your care team, as well as coordinate necessary care with specialists.
Hospitalists typically complete residency training in internal medicine, pediatrics or family practice and may also be board-certified in Hospital Medicine.
FAQs and Benefits
Your hospitalist will manage and coordinate your comprehensive treatment plan, working with providers from other services to deliver the care you need — including nursing, dietary, diagnostics, pharmacy and specialty care. He or she provides these essential services for your hospitalization and optimal health outcomes:
- Coordination of care and treatment during your hospital stay
- Collaboration with your primary care physician and specialists as appropriate
- Ordering of diagnostic imaging studies and lab work
- Review of test results and appropriate care plan adjustments
- Order, management and monitoring of medication changes or additions
- Communication with you and your family on care plan updates and questions about your illness, medications and other planned diagnostics or treatments
The hospitalist’s primary focus is the coordination of your care and services in order to achieve optimal health outcomes. He or she will develop a comprehensive treatment plan and work with providers from other care disciplines to deliver needed services. Upon discharge, your care returns to your primary care physician and/or follow-up care with specialists as needed.
Patients served by hospitalists often experience a shorter hospital stay by between 14 and 17 percent, compared with those followed solely by their primary care physician or non-hospital medicine specialist. The healing process continues post-discharge in the comfort and privacy of your home and under the care of your primary care physician and/or other specialists.
